First. A foundation is not a bottomless bag.  Each foundation, in addition to being governed by its own rules , has a specific pool of money. She will not get into debt and will spend as much money on support as she can or believes she can.

There are different and individual procedures between foundations, so I will give you the most likely scenarios on how to proceed if you want to take advantage of their support. You can read everything on the foundation's website in the rules and statutes.

Documents must be completed; What?

​

• Certificate of disability, in accordance with the original or the original available for inspection.

• Application from a doctor to buy a wheelchair

• Please indicate the income per family member (don't hide anything)  from all places of this income, on average per month.

• Consent to verify the information provided in the application

• Photo of the applicant/sometimes legal guardian.

• Consent for the processing of personal data.

• Photocopy of all support, purchase or refusal documents from other institutions. PFRON, PCPR, NFZ.

• Sometimes fundacja ask for information on how money for the cart is collected. (collections, transfers or other foundations)

• You should always write a justification for such a request for support, describing the situation and the need to buy a wheelchair. What does it change in the life so far or in the form of therapy. You must always sign as the applicant or guardian.

The Foundation always states when it will consider the application and it really varies. A few days to 2 months.

Sometimes you also have to reckon with the fact that you will have to set up a sub-account  1% in this particular foundation, but this is not always the case.
